Adnexal Masses


We begin our discussion of adnexal masses today. A 73-year-old woman is referred to you for evaluation of an adnexal mass. She had been seen in the local emergency room following a motor vehicle accident. Abdominal-pelvic CT scan revealed a 3 cm left-sided adnexal mass. She is asymptomatic and in excellent health.

  1. Should this mass be removed?
  2. What is the first step in further evaluation of this adnexal mass?
  3. What other diagnostic tools would be helpful?
  4. Should you order a serum CA125?
  5. Your evaluation reveals a simple ovarian cyst with a CA125 of 12 U/mL. Are you obliged to recommend surgery?
